After receiving penicillin intravenously, a 32-year-old male patient has an anaphylactic reaction. The nurse understands that therapeutic management includes what critical items?

In an anaphylactic reaction the throat swells shut form the allergic reaction. When this happens the airway is cut off...so critical management is the basic ABC's of life support.
1. Airway ...manage the airway...if its not open he cannot breathe ...may require intubation
2. Breathing ...is it effective breathing or compromised breathing...if he is in distress..again...intubation and probably mechanical ventilation....until the anaphylaxsis is
reversed.
3. Circulation...if the airway is compromised long enough ..the heart will stop...respiratory arrest. May require CPR ...if patient is already being managed..check circulation in distal parts of the body.arms fingers..legs and toes..make sure they are warm and within the patients normal coloring...not bluish, gray or cold.

You would first stop the infusion. Manage the patient's airway with oxygen or if needed intubation. Medications of Epinephrine, Benadryl, Solu Medrol, would be given. You would then manage care as to how the patient responded to the previous interventions.

May need a breathing machine (ventilator) epinephrine, 25 to 50 mlgs of diphenhydramine, IV or IM. IV fluids, resp nebulizer, large quantities IV fluids to help with bp, also epi, norepi or dopamine can be given. Also steroids can be useful

ABC + corticosteroids and if required adrenaline subcutaneous injection.
Hospitalization for at least 24 hours.
