There is no such thing as a 100% reliable method of contraception, other than not having sex, but condoms are something like 98% effective if used properly.
Yes, there is a chance a condom could tear or rip or slip off, so you do need to be aware of that and follow the instructions on the packet if you haven't used them before. They prevent pregnancy by stopping sperm entering the vagina.

Condoms are about 80% effective at preventing pregnancy when used correctly and more so for protection against STDs. The correct use of a condom is NOT to keep it in a wallet, where your body heat will decompose it, to put it on as soon as the penis is erect (as pre-ejaculate containing sperm can leak out prior to orgasm), to only use water based lubricants like KY or some of the newer brands, NOT baby oil, vaseline, or any lotions or oils, and to remove the penis from the vagina right after intercourse and remove the condom.

With my experiance condoms have worked yes.
And with "Ripped off" You mean them comming off? Ya thats happened to me before while having sex, lol
It does help avoid pregnancy, and help prevent STDs! :)

to this what gahrahstah said i would add only that it is good to buy condoms with spermicides, if you want increase safty you can also use spermicides in foam or gel, and put it in to your vagina (i think near cervix, its near the uterus if i good remeber,) about useing correctly foam and spermicides the best talk to gynecologist as when you start sex life you should go to one, at least once a year to do pap test and some other.

Yes, they are pretty reliable.
Just never use any baby oil at the same time because that can cause them to tear. (Happened to me more than once).
