Stop smoking.

Thallium scan is a nuclear medicine scan looking for any areas in your heart that may be suffering from decreased oxygen (called ischemia... hence ischemic heart disease) from blocked arteries from high cholesterol (?insufficient HDL)/smoking/high blood pressure...

If the thallium scan shows an area of ischemia, you will probably need some medication to thin the blood, lower your LDL and dilate your coronary arteries. (You may also need an angiogram.)

Holter monitor is a 24hr (or sometimes longer) recording of your heart electrical activitiy. This will detect if you have any irregular heart beat electrically during the time you have the holter on... some people get palpitations through anxiety/stress rather than an actual irregularity in the heart beat. If an actual irregularity shows up, this may account for the flutters and shortness of breath. You might need medication for it.

The more information about your heart the better to decide what your treatment options are. You may benefit from medication only, or you may require a surgical procedure.
