Diuretic of the thiazide class. One of the mainstays of treating high blood pressure for decades. It forces the kidneys to remove fluid, decreasing the volume in the vessels, decreasing the blood pressure. Some groups are much more responsive to treatment with hydrochlorothiazide than others. It is probably underused.

Advantages:
Well known and well understood
Available as a generic at an absurdly low price
Effective as a single agent in many early and mild cases
An excellent added medication to other anti-hypertensive drugs

Disadvantages:
Causes loss of potassium and magnesium which very often have to be given as supplements to ward off problems
Often has to be part of a multidrug regimen except in relatively mild cases.

Hydrochlorothiazide is a thiazide diuretic (water pill). It decreases the amount of fluid in your body by increasing the amount of salt and water you lose in your urine.
• Hydrochlorothiazide is used to lower blood pressure and to decrease edema (swelling).
• Hydrochlorothiazide may also be used for purposes other than those listed in this medication guide.

Hydrochlorothiazide is a diuretic agent belonging to the class of thiazide diuretics. Its primary indication is as first line drug for control of hypertension, either alone or along with some other agent like ACE inhibitor, Beta blocker, etc. Its a weak antihypertensive by itself, and it potentiates the action of other agents. Its effects include increased frequency and volume of urine. It acts by blocking reabsorption of sodium and hence water in the distal collecting tubules of kidney nephrons.

It can also be used for diuresis in conditions like congestive heart failure, though it is not an ideal candidate for it. Its also used for central diabetes insipidus . Again not the drug of choice.

Its side effects commonly include electrolyte disturbances, most commonly hypokalemia (decreased potassium in blood), and hypocalcemia. Also it leads to hyperglycemia and increased blood lipids. It should hence be used with caution in diabetics and dyslipidemics.
