Do you know good manufacturers who can create custom molds?

This would depend on the volume of parts you want to create from this mold. On my site I have lots of information about this. It also depends on what stage of development you product is in. The size of the part, the material it is to made from. The process varies widely, I have made rubber mold and cast parts from urethane that are plastic like. They work OK for a handful of parts. You'll need a master or pattern to cast from. These can be hand made carved or produced using rapid prototype methods from a CAD data. Aluminum molds and mold inserts can be also used. These molds are capable of making hundreds to thousands of parts by injection molding. Thermoforming may be another option. If you want a production mold capable of producing thousands to millions of parts a steel mold might be what you need. Feel free to contact me or visit my site for more information.

contact tool & die shops, ask if they make injection mold dies.

Most tool and die shops can fabricate one, but they can be pricey!
