When addressing wedding invitations, titles such as Mr., Mrs, Ms, Rev, Dr, Maj, Sgt are all abbreviated. Street, Avenue, Boulevard, North, South, and the like are to be spelled out as is the state (Washington instead of WA).
If you are still unsure, go to your local library or bookstore and look at wedding ettiquette, most books will have examples for you to follow.

If your wedding is a more formal affair or even if they are formal looking invitations you should spell out all the words in the address, street instead of St., Southwest instead of SW. Your invitations set the tone for a wedding and help imply to your guests how formal it is going to be.
Doctor is a profession. Dr. is a title. When addressing someone you abbreviate, just like you use MR. instead of Mister.

Spell out as much as possible on the address portion of the invitations. Even if the wedding isn't formal, it still gives the mail class. Dr., Mr., Mrs., Ms., Rev., Sr. are all acceptable abbreviations. I suppose it wouldn't hurt anything if you did spell them out. Hey, better safe than sorry.

Ask your friend how formal to make those things. Dr. is appropriate, never doctor. As for the rest, it use her personal prefernce as a guide.
