I have this new 14 month old adopted little sister, and she has a birthmark onthe left side of her eye. Now if a little kid asks what that is what would you say instead of telling them that it is a birthmark? I am just curious.

When you don't tell kids the truth they learn not to trust. When they find out the truth they can't always figure out why they were lied to. Of course if it's a really little kid 2 -3 they might not know what you're trying to explain anyway.
Older than that 4 - -> tell them the truth. Explain that most people have marks they were born with that sets them apart from everyone else. Otherwise all our bodies would be alike. This is hers and it's rather special because it's difficult to miss.
If you act like you don't want to talk about it, kids will also get the impression that there is something wrong with it.

You want to tell the truth for your sister's sake. If you lie to other children about what it is then your sister will grow to be ashamed of it because you are trying to hide it from others. Small children understand a lot more then you think. Don't give her a complex. By telling the truth you will also be educating other children not to be so prejudice against children that are different. If you don't act like it is a big deal then children won't either.

What is wrong with the truth? My nephew had a strawberry on his neck, which faded over time and my sister used to tell children it was an angels kiss...I like the truth. It is a mark the baby was born with and it will go away some day (if it will).

oh gosh there are many things you can say but before you say it to anyone discuss with mom and dad - if you bring her up thinking its no big deal no one will ever make fun of her,
you can say
our grandfather was a (fill in the blank)
giraffe, cow, leopard whichever fits the mark best
oh that's God's thumbprint when he blessed her
give it a light funny spin,
angels were eating chocolate when they kissed her
after today never call her adopted again - from now on she is only your sister - it will make her and you feel good

i have a notacible birthmark on my forehead and everyone thinks its a burnmark. it depends what it looks like_ u can say its dirt, a bunmark, marker, or if its a little kid, just say shes got a boo boo

WELL ALSO HAVE AN SISTER AND SHE HAS A MOLE IN THE CENTER OF HERLEFT CHEEK AND ON THE LEFT CORNER OF HER FOR HEAD SO SHE COVERS THE ON HER FORHEAD WITH HERBANGS AND SHE SAYS IT'S MY BEAUTY MARK SO BACK OF SHE'S CRAZY AND SHE'S ONLY 11INA HALFAND SHE AFRAID TO MEET CUTE BOYS CAUSE OF THAT!!
