The only way is by oral medications which in your case may still be indicated in-spite of interaction with other medicines. Look up sporonox and lamisil. Talk to a Dermatologist and get follow-up advice from your Primary Doctor.

Your case have 2 possibilities:
1- Either you're not constricted to your daily diet & insuline, which then cause an increase in your sugar level. Hence, your immunity can not defence aganist your fungal infection properly.
2- Or your diagnisis & management of your nails are not match.
