WARNING - Grossness ahead!
Dogs have two structures called 'anal sacs' near the anus. They're similar to the scent glands of skunks. Small glands lining the sac produce a smelly brown fluid which is stored in the sac. When the dog has a bowel movement, the sacs are squeezed and the fluid gets into the poop - that's part of what gives dog poo its distinct odor. Dogs may also express their sacs (i.e. squirt out the liquid) if very frightened.
Anyway, in some dogs the sacs don't get emptied properly during normal BM's and the fluid builds up; the sacs may even become impacted or infected. This is uncomfortable for the dog, of course.
A lot of animal care people will routinely express the sacs while doing other care. This is done by gently squeezing the anus from either side and drawing back slightly to 'milk' the fluid out. In some cases the sacs need to be expressed individually, which requires putting one finger in the anus and the thumb outside. (Wear gloves!) Some dogs tolerate this better than others.

Incidentally, they're usually called 'anal glands' because 'anal sacs' sounds like something naughty..

this is clensing the anal gland which can be uncomfortable for the dog if not done. The best thing is to take him to a groomer or vet and get them to show u how it's done. Once u've seen it, it couldn't be easier to do

I've seen it done with horses when they have colic there's an injection that makes them poo and I've seen a vet stick a hose pipe up his behind and sticks water in and then the poo comes back out with the water. It's to clean their bellies out I think and to prevent colics.

Cleansing the anal glands is a normal procedure that groomers tend to do, and some people take it upon themselves to do it at home. Veterinarians may do it as well.
