Some people call it a fetish, but a fetish only develops when a person becomes sexually mature (round 13 years old) but Adult Babies mostly have had an urge for nappies since they can remember. There is a sexual link once they become sexually mature and that is why people can't distinguish between the two.
Find out more from your friend what he requires. Tell him to be totally honest with you. Let him paint you a scenario of what he wants.
And remember about 5 to 7% of the population likes to wear nappies, for whatever reason. And about 20% of adults and children have to wear nappies for medical reasons, but they are not considered Adult Babies.

I think it's a way to for one caring person to submit to another. In a way, the AB is surrendering himself/herself to the person he/she cares deeply about.
If you think about it, it is a gesture of complete trust. Trust that the person being submitted to will not take advantage of or reject him/her. Trust that the person being submitte to will not cause harm to the one who is trusting.
I only wish I had the guts to trust that deeply.
