It means they have had to Dig it up for some reason. (in many places in Europe Graves are LEASED and not sold so after about 50 or so years they Exhume them and burn what is left and get rid o f the ashes. Exhumation is also used for Forensic purposes.

Following exhumation and examination the body if finally re-interred with appropriate cermony conducted over it.

if it's exhumed, it's been removed from the ground after it's been buried. Police and grave robbers sometimes do this.
