is it appropriate that everyone (males and females) stand up when a woman leaves the table or is it an etiquette that only the guy should stand?

What is the proper way and why?

All men stand at the table until all the women are seated.
Once all the women have been seated it is customary for the men to raise if the woman leaves the table. But to be carefull to make sure that she has a clear path to walk.
Once a woman returns it is acceptable to have a gracious raise. To leave the chair were it is but raise to a 5/8ths standing position.
Once a woman leaves the table to for the last time, all the gentlemen at the table should stand.
And yes, this is still in practice. My mom would backhand me if I did not show proper respect.
(but never at the table of course)

Initially I think, it was something a Gentleman does when a Lady stands or sits. However, if you really Honor the person in question, why not everyone? For instance, the President, everyone might stand for him/her.

Only the men should stand, unless you're the one married to the woman. Then just ignore her.

The boys stand and the girls stay in their seat. The boy she's with should be the very first to stand and and pull the chair for her.

In a group, nobody should stand. I dont even think it's necesary to do it if it's just a man and a woman that are dining alone.

Normally it's just the guy that's with her & he pulls out her chair for her, which is why he has to stand.
