Ice pack on the back of the neck. 30 minutes on then 30 minutes off. Repeat as necessary. Preferably lying down in a dark quiet room.

i have migraines, i have gone to the doctor and he put me on a medication i take at the onset of a migraine- it is Midrin, and works well for me, although what works for me may not work for others. i also avoid caffeine and other things that trigger my migraines. i have figured out what a few of my triggers are and try to avoid them. some of my migraine triggers are cigar smoke, different perfumes or colognes, smoke from clove cigarettes, extended time out in the heat, extremely loud noise and lights (like at a concert)
some other things that work for me, when i'm having one- darkened bedroom, no noise, cold washcloth over my forehead and eyes and another cold washcloth at the base of my neck. elevate my head, and they usually go away w/o taking my medication, but not always.
