I feel for you. I am disabled and collect Social Security.
You can start the process on line at: https://s044a90.ssa.gov/apps12/best/benefits/
Use the BEST (Benefit Eligibility Screening Tool) form to see if you can qualify. You also need a doctor to agree that you are eligible for Social Security Disability. If you get past that then you will probably need to go to a Social Security Office to apply. Find you local office at: https://s044a90.ssa.gov/apps6z/FOLO/fo001.jsp

You will automatically be turned down on your first application. You will then need to wait and reapply. You could try and talk with a lawyer after you get a doctor's written statement saying that you are disabled.

Check the phone book, there are a lot of lawyers that only handle cases like yours. For the hearing you will HAVE to hire a lawyer. They take their fee (up to a max of $4,000) from your benefit settlement (a lump sum payment, not your monthly checks). They get paid directly, you never see their fee. If you have a witness that can say how you are disabled, and why you can't work that will help a lot.
