People who are morbidly obese have very poor circulation in their extremities. Because of this, blood pools there, and when it dies it disn't flushed from the body via the kidneys.
This dead and decaying blood then causes the infections.
This is exactly the same thing as a bedsore. In fact, Christopher Reeves died form a bedsore. The infection spread to his heart, which triggered his fatal heart attack.

when the skin touches skin with no air getting in it can cause sores. Im sure there are other reason/causes. Sometimes the hygene is an issue

Poor circulation is the main cause. There can be other conditions such as diabetes. These are the two main causes I have seen.
