Do you want to give your doctor a card?? You can print out a card made from Print Shop or any other card making program, and have your child color it. You can also add a picture of your child to make it more personal.

The best way and what will be appreciated the most does not come from printed materials. It is better for your child to draw something to give to her doctor, that by itself is special and her doctor will surely appreciate it.

The American Medical Association has cards you can print out thanking your doctor. You could print it and let him color or decorate it

You are going to have to be more clear in your question - do you mean you want your child to send him a "thankyou" not or a note saying he or she likes him?
Or do you mean a note explaining how your child is feeling?

If your child is a preschooler, have him/her draw a picture. You should clarify your question better.
