I am not sure what you are calling the "ULTIMATE SIN" but if one of my brothers where doing something illegal and I knew that what he is doing is hurting other people I would have to say that YES I would report him.
Many people hold to the old adage that "Blood is thicker than water" I agree there times when you should be loyal to your family. If some one is taking an advantage of them or stealing money from him than I would say yes be loyal and help him.
In this case I feel that you are protecting someone that is very dangerous to others or at least himself. I encourage you to report him to the authorities or if there is some where you can offer him some help.
It is your brother but if he is doing something illegal and you don't report it you can be held and charged as an accomplice.

If the ultimate sin is messing with children then yes report him to the authorities and they can get him some help. If he is contemplating suicide call the mental health people in your community and get advice on what you can legally do.

In the real world, only CRIMES need to be reported to "the authorities". Do you mean the police, the Immigration Service, or what?

What authorities? The Spanish Inquisition? If you mean the police, they only address crime, not sin.

I would probably talk to him first and try to get him to see the error of his ways. But yeah: if he refused to repent, and his actions were sinful and criminal in nature, I would turn him in(*). Otherwise I'd be part of the problem, right?
