si, it does mean that, but there are different variations of spanish just like in english. sometimes what someone says in venezuela is not the same in mexico.

"¿Qué estás haciendo?" does mean "What are you doing?". Maybe this girl wasn't doing anything good, so she felt ashamed.
Anyway, in certain contexts, "¿Qué estás haciendo?" may be taken as "What are you up to?", so it may sound a little judgmental. Maybe that's what she interpreted out of your question.

Yes, that's what it means in 'perfect' Spanish. Slang could be, 'Que pasa' (Wassup, What's happening, etc.)
