depends on where she is in her monthly cycle. She could go get emergency contraception or the morning after pill.

Everyone makes mistakes and I mean Everyone.. She was very young and very immature when all of this happened. She has trusted you with the truth because she believes she can and is hoping that you can somehow understand. I do not believe this girl should be judged on her past behavior. That is over and done with. She learned a very hard lesson and beats herself up over the past. If you love her then tell her how you feel and find a way to move on. Put yourself in her shoes. Would you like to be judged and lose something very important to you because you made mistakes in the past? Before you matured enough to realized your behavior patterns were self destructive? The other thing is , you have no idea what drove her to that. Problems at home ETC... It is very obvious that she did not have much self esteem at the time. There is always a reason for that. Keep all of this in mind and ask yourself is it really worth losing her over? Best of luck to you and I hope that this can all be worked out.

That is exactly how girls get pregnant. That is why condoms are only 99% effective. There is always the chance that one can break. More important is the risk of STDs being transmitted. Your friend needs to make sure that if she starts having any funny or strange discharge that she goes to a doctor, she could have an STD. Recently they have linked the human papalona virus, an STD, to cervical cancer.

A pretty good chance if she's ovulating during that time.
She could get emergency contraception if it's been less than 5 days since then, so she won't get pregnant.

depends on when her last period was/whether she was ovulating at the time; but even if she wasn't ovulating, sperm can live for about 5 days so there's always a chance

That depends on a lot of things... how fertile he is, how fertile she is, whether or not she was ovulating at the time, how much got in, etc.
