The peloton (pack of racers) decide on a spot to pull over and wee in the bushes. The final say is usually decided by the leader of the race at the time, the person wearing the yellow jersey. They all stop together and go, and cycling "etiquette" deems that no one's allowed to sprint ahead while the group stops. If a racer has to wee later, well, then he just has to go or risk getting dropped.

there are usually specified stops on the route where riders can stop and go. If there is a big gap and they are far anough ahead they can go by the side of the road. There is a very famous picture of the cyclist Bernard Hinault looking round as he wees off a bridge.
In the last 20km then usually they try to hold on, as if they are needed at the controle anti-dopage (drug testing), then they may be required to give a urine sample, as well as blood samples

Most cyclists will not wet themselves in their shorts, it creates too much chaffing. They do stop more often then not. It's actually considered impolite to launch an attack while the yellow jersey is taking a pee break. Did you hear about the guy who used to always attack on the pee break? The peloton got so mad, that when he stopped for a pee break, they grabbed his bike and dropped off about a mile up the road! That will teach 'em.

One option is to hitch up the shorts and have a team mate help steady you while you go. If you have a big enough gap in the break away, you can make a quick stop. If enough riders have to go, the peloton will slow up while the riders stop and resume the attack once the riders are back with the main group.

If you watch the broadcast carefully, you will occasionally notice riders stopping by the side of the road to relieve themselves. This is common on the flats. In the mountain stages where you do not want to lose the peloton, it is more common to let yourself go in your shorts. For those times where you have to do more than just pee, there is no choice but to stop at the side of the road.

usually they all stop at the same time. there is a photo of this somewhere, but not sure where.

some will stand up on the bike while riding and a teammate holds their shoulder and the go while riding.

some go in ther pants depeding on how critical the race is or how close to the finish.

let it rip, i used to race and it comes as a great relief when you are racing out in the cold, the heat gives you a boost.. never won much myself like....

those answers are semi true, you can hitch your shorts up and not mess yourself totally...
