Get your Birth Certificate and GO to (do not email and do not call) a Social Security office to get a new card. There are forms there to fill out .They usually don't ask for anything else in person.

Most of what everyone has said is PARTIALLY true. Some of the previous remedies will work but it is very frustrating. The truth is that, in California, I was informed by the DMV personnel that I needed an actual Social Security CARD to show them in order to get an official state issued ID. Now keep in mind that they verify that the name matches the number electronically while you are standing there. They don't even use the card for anything...just the number. After a few choice words and criticisms that my 18 year old had NEVER heard me utter I went back to the SS office. We had already been there and were told that we needed a photo ID in order to get the SS card replaced. VERY LONG STORY SHORT...we were allowed to use a medical insurance card without a picture to obtain the replacement card. When we returned to the DMV with the SS paperwork, they said they didn't need it...only the number. ID card was issued. HOWEVER!!!!, my daughter then attempted the same scenario to replace her lost documents after a move and the entire situation has repeated itself. She cannot apply for a job or state benefits and is now in the predicament of having no money and no way to make it except cash paying jobs here and there. It would be wonderful if someone in authority or someone in the legal profession would help rectify this situation so many have had to deal with due to the inept policies of this bureaucracy.

You need a copy of your birth certificate. If you don't have a copy of your birth certificate go to the Hall of Records where they issue Death and Marriage certificates in the State where you were born. If you don't live in the same state, you can get a copy by mail but it will take longer. The Hall of Records will provide instructions as to how you can get a Birth Certificate replacement.
Then you have to go to the Social Security office and apply for a replacement card.
It is quite a process, I had to do all this to get a passport, but it will be well worth it once its done.
Just be patient.

You can contact motor vehicles and tell them that you only have a birth certificate and be able to recieve an identification card, then you can use that to get your social security card, do you know the number? If not you may be issued a new one. If you have your birth certificate thats a start you may have to call around, but at least you have one thing, they may even take a school year book as proof if you get desperate

All you should have to do is fill out the form for replacement card at your nearest social security office. Bring along your birth certificate. They can check and see if you were issued one and since you were they can process the request right there.
Maybe if you have them take some old bank statements or old pay stubs, or an ATM/Debit card along.

This happened to a co worker of mine. He had to bring his work ID, Birth certificate, Utility bill and yearbook that had picture of him with his name. He did it, but it took him 4 times to get it right. If you work, your personnel office should be able to provide you with documentation that you work there too.

Go to your local social security department, i had to do it a while ago. Just go to yahoo yellow pages and type in social security department, there should be one somewhat close to you. Good Luck

My daughter lost hers. She had to have birth certificate and got another one.
