This is not poetric question. I have read many verses in Quran that say people "think" by their hearts. An example is Hajj [22:46]: "Do they not travel through the land, so that their hearts may think and understand or their ears may hear?".

I ahve always thought that all those verses are just poetric, until I found two things:
1- I found that all early commentaries (Ibn Katheer, Tabari...etc) confirmed that people "think" with their hearts.
2- I found an authentic prophetical statement told by prophet Muhammed the following:
"Angel Gabriel came to him and took his heart out of his chest, cut a little piece from it and threw it away saying that this little piece is the evil part of his heart, washed his heart with holy water, and put it back in Muhammed's chest. Since then, prophet Muhammed used to show his companions the mark in his chest with the stitches on it"!. This statement (Hadith) was narrated by most reliable narrators [Muslim: 236, Musnad Ahmed: 13555 & Nasiee: 448)

Is it true that people "think" with their hearts? and is it real that we can have a surgery to take the evil out of our hearts?

The definite answer may be come from the medical science in future. God says that we think from the heart will be proved to be true, because the emotions create thinking and I believe heart control all our body and mind would actual do the thinking work. The hadith regarding Prophet Muhammed (PBUH) can't be true because Angel Gabriel never did appear physically infront of him.

The interesting part is in your additional comments which says that we go through the heart transplant and get the evil out of us. Dear friend I found that this is what happening with lots of people who had a heart transplant.

Some heart recipients report strange changes

Actually the mind is what thinks not the brain.
And the mind is a state that can not be touched or looked at because it is composed of emotions and etc.

People believe a heart makes them love or show other emotions; that is not true for that is the minds job. But by heart, I think, the Prophet Muhammad (pbuh) meant really the part of emotion. And as we do bad deeds we lose the emotion of guilt and etc. So by cleansing His heart they must have washed away the little evil from his mind.

Also I'm sure you heard of the Frontal lobe, which inspires bad thoughts because it is the only voluntary part of the brain. It is the making choices part if you will. But also Satan whispers in it and it is our choice if we listen. And this also explains why the brain (frontal lobe) is not best for thinking and it also explains why Muslims prostrate (to rid the words that Satan whispers into their minds). All of this info can be found in the Quran.

That is very true, Quran is written in a way that no human being can ever write.
Arabic is very poetic language, this what Arabs call BALAGHA ( eliquancy in the language).
Some meaning in the Quran need a person who is perfect in Arabic to understand and explain. Perhaps this is why some people interpreter the Quran in different ways. That was why Muslims never liked translating the Quran.
I never heard about that Hadith you mentioned before..!

Be careful of all hadith (Prophet Muhammad's (SAW) sayings and actions) b/c it is word of mouth; there is no Sahih (authentic) book except the Holy Qur'an. Try tafsir al-Mizan.

Many people think by their hearts but not all, and the same applies that many do not think with their hearts but not all.
Thats like saying all Americans like American food, most do but not all American food, you are always going to have a split ratio.
