he is probably in Somalia again he has strong ties there. If your old enough to remember what happend down there the last time he man has to be close to a hospital due to his health

Because he is hiding in one of the most remote and backwards places on earth.
Also - capturing him will have little to no strategic value so we are focusing our resources on things that have a higher payoff.
