Usually that means that someone (a doctor/a nurse/a person's care-giver) checks certain aspects of a person's health on a regular basis. So a parent might do sugar-level testing for a diabetic child. Or a wife might take her husband's blood pressure on a regular basis. You can do your own monitoring if you want and you're healthy enough to do it. "Monitor" means to check.
