I have both problems and it can be very confusing trying to tell what is what. I have been a diabetic for more than 40 years, and the carpel tunnel came first (at age 30+) with numbness in my wrist that traveled down to my fingers, and sometimes to my forearm.
Severe neuropathy came later, but I think that the conditions overlapped with me. I had surgery on my left wrist (the worst) for the carpel t. combined with surgery for Dupuytren's contracture on the same hand. Big mistake--months of healing and therapy, but it did help the carpel t., for a while. I now have severe carpel t and neuropathy in the right wrist and hand. I can't hold on to anything; always dropping things;can hardly write or draw (my passion lost). I can tell when it is my carpel t. acting up -- I grab my wrist with my other hand and squeeze hard and flex my fingers -- instant relief -- for a short time, that is. Neuropathy causes me severe numbness in my finger tips. Thinking of surgery, but remembering the past surgery problems. My experience: Try therapy for the carpel t. before surgery -- We diabetics are hard to heal. Take better care of your sugars -- The neuropathy can only get worse, never better.

Carpel tunnel is a nerve compression (meaning your nerve has swolen due to over use of the hands) and neuropathy is nerve damage, it mostly starts in the feet and can work its way up to the hands.
I had Mortons Neuroma in both feet which is also a nerve compression. I also have neuropathy, neuropathy is much, much worse.
Carpel tunnel can be helped by wearing a hand brace. No brace will help neuropathy.

Carpel Tunnel Syndrome is a repetitive strain disorder and affect primarily particular nerves and usually as a result of certain types of occupations, where there is repetitive activities, such as typing, food preparation, using a cash register or in certain kinds of light manufacturing. CTS is a compensable injury under most workplace compensation schemes and can be diagnosed by an EMG test issued by a neurologist.
Neuropathy is more slower in coming ... it is a pain that cannot be attributed to any of the above activities. There is also a numbness and loss of sensation associated with neuropathy that can lead to difficulties such as nerve damage, infections, foot ulcers and other complications.
This is only one more reason Diabetics have to ensure their glucose is under control and they receive regular medical monitoring of their condition, as well as take care of their extremities (particularly in extreme temperatures or around the house where one can step on something that can lead to an injury).

Carpal tunnel syndrome comes from the compression or pressure on the nerve. Although it can happen due to fluid build up during pregnancy, usually it is a result of chronic, repetitive actions (like typing).
Diabetic neuropathy is a nerve pain that comes about from the damage to nerves caused by the chronic elevated blood sugars.
Your doctor may be able to give you a good idea by an exam, but having a EMG would probablly be more definitive.

Carpel Tunnel is a patholgy where the Median or Medial Nerve is compressed, this causes pain and weakness in your hand and a Neuropathy is a condition in which peripherical nerves are afected due to the diabetes, nerves transfer signals to the muscle to make movements this mechanism uses proteins and neurotransmisors which are afected by the high levels of Glucose in the blood by altering their properties.

Carpal tunnel syndrome is the compression of 'median nerve" at the wrist.Carpal tunnel is the imaginary groove between wrist bones and a band like tissue called the "palmar facia".This gets narrowed in diabetes because of deposits.Median nerve supplies nerves to skin and muscles overlying little and ring fingers of the hand.Surgical decompression cures the condition.Diabetes controlled will not reverse the condition but prevents a second time.
