Fear of success? You'll be graduating soon. You'll have to start facing an adult world and that's kind of scary, isn't it? It's all well and good to say you've tried to set goals, but that hasn't worked because the goals may not be reasonable. Why set your self up for failure by saying you'll get it all done at the beginning of the week? Why not say, I'll get all the math done on Monday, all the history done on Tuesday, etc.
Then too, you may just have a personality that works better under stress -- if it's due by 3 PM, you work your butt off, starting at 2 PM.
The problem isn't that you're failing in school, but rather that you think that you're failing yourself. You seem very down on you. Perhaps if you decide what career path you're going to set out on, you'll have a reason to study and a reason to feel better about yourself. Is that what you're trying to do by all the daydreaming -- imagining your future?

Make sure that there are no outside distractions, such as TV. Then it is a good idea to take short breaks in between study sessions. Walk around, do some exercise of some kind to wake yourself up. If the internet is a big distraction than try to stay off it as much as possible, even shut down the computer if you need to. Also, if your parents are around, tell them to check in on you to make sure you're studying and doing your work. They can help you by keeping you accountable.
It is also a good idea to not study one subject for too long a period. This leads to boredom and then distraction. If you switch between subjects at shorter intervals, then your brain is forced to concentrate.

4 am. I refuse to pull an all-nighter, no remember how a lot analyzing I truly have. yet yeah, i have stayed up until eventually 4 cramming...i became delusional the subsequent day. that is not any longer uncommon for me to be up until eventually a million:30 if I positioned off a project too lengthy or i have were given a needed try the subsequent day. I freaking hate extreme college. i comprehend college will be worse...yet ugh. BQ: It ends Tuesday. the following day is my very last time without work. BQ: ok. i did not somewhat do something particular. besides the undeniable fact that it wasn't undesirable, i wager. merely no longer awesome.

Try setting a timer for 20 minutes. Work as quickly as you can for those 20 min. and then take a break (time that too or you'll take too long). Then set the timer again. You'll have your work done in no time.

the ipod...
i use mine all day during school, i never hear a word the teacher says i just sit there listeing to my music and reading out of books, and i have strait A's
and i do the same thing at home..it helps drowned out the outside world so that you can focus, and music is a powerful memory tool

You need to sing or at least hum. Compose nonsense songs with words that don't exist, and sing them as you study. Loudly at times. When you get tired of that, hum your songs. Good luck, and let us know how it goes.

You can try clasical music in head phones. That helped me when I was trying to focus.
