After DNA replciation in Mitosis, a chromosome consists of two identical strands, or chromatids. These strands are then seperated during Anaphase to for the new daughter cells.

A chromosome is a condensed version of DNA. If DNA in a cell was unwound, it would be very long. Therefore, it needs to be packaged into a smaller shape so that it can fit into the nucleus. So, DNA is wrapped around proteins (known as histones) in order to form a more condensed structure, called a chromosome.

They are both one in the same. Chromosomes are tightly-coiled strands of DNA. Normally the DNA is dissociated in the nuclear envelope and is just called DNA. However, before cell division, DNA becomes visible to the eye (simple light-microscope) in the tightly-coiled chromosomes.
