I think Arizona does allow medical benefits for unmarried couples, because a proposed amendment – “Protect Marriage Arizona” – would limit or end this. It includes the language “no legal status for unmarried persons shall be created or recognized by this state… that is similar to that of marriage.” So, even though the amendment was proposed to end gay marriage (which is already forbidden under Arizona law), it would affect unmarried straight couples, too.

Does Arizona have domestic partner agreements? If not, I doubt it. Check with the insurer.
