It's depend of wich creole.
The haitian creole: "Mwen renmen ou"
I am from Martinique, so creole is one of my native language. The creole is slightly different depend on where you come from, even in the same country it mays differ from a part to another.
The creole from Martinique says: "Mwen immin'ou". The creole from Guadeloupe says: "An immin' vou".
"Je t'aime" is french.
You need to know the pronounciation too otherwise she won't understand you.

As everyone is saying there are several creoles spoken, so depends on where this person is from. I can give you 2.
Creole of Reunion Island: "Mi aime ou."
(pronounced me-emma-oo).
Creole of Mauritius Island: "Mo content toi."
(pronounced moe-conton-twa).
These islands are part of the Mascareignes in the Indian Ocean, in case you are not familiar with them.

There are hundreds of different types of french based Creoles. I know Dominican creole but it's propbably not what you're looking for.
Be more specific.

'Mwen renmen ou' is the correct way.
The first answer is actually french not creole, and it's "je vous aime". People often get mixed up between creole and french because creole is a french based language.
