Shortness of breath with upper back and throat pain sounds like heart problems.and these symptoms are very serious. Your boyfriend needs to see a doctor immediately. Sometimes heart problems that are congenital (ie we are born with them) do not become noticeable until a person is in their teens or twenties. Undiagnosed heart problems can cause sudden death in young people.
Tell him about this. How do you feel about telling his parents? He may be very angry with you but this might just save his life. They might be able to get him to see a doctor. If he still refuses to see a doctor and starts having symptoms again call 911.

Hard to figure out whats going on by this description.

If these symptoms just recently occured, i would see the doctor. It could be strep throat and people can get fairly sick from it. Treating strep throat is important because if left untreated people can develop rheumatic fever.

If he has been experiencing these symptoms for a while and if it has been get worst and if he has "heart burn" (GERD), he made be refluxing acid into his throat and lungs. People which this severe type of acid reflux can develop back pain, sore throat, and SOB (especially is the acid gets into his lungs).

So go see the doctor and figure out whether this is an infection or something else.
