I see this on a note on an engineering drawing of a pressure vessel attached to the drawing of square nuts on the vessel

12" centers both ways is in relation to whatever it is you are doing to the piece. for example, if it is holes the holes will be from the center of the first hole to the center of the second hole 12" apart. left and right measurements are your "X" axis, and up and down will be your "Y" axis There should be a measuremnt to the first hole, then you would go from the center of that hole 12" in either direction to achieve where your second hole would be.

Perhaps it means that the "centers" for a row of studs (bolts) and nuts are twelve inches apart. Were holes drilled 12 inches apart? The terminology must be applied in context. Good luck.
