In medical terminology trans means through or across. Think of the word transparent. You can see "through" something. Femor means femur which is the largest/strongest bone in the human body( your thigh bone). -al means pertaining to. When the term femoral is used, it's most likely the artery which passes the femur and groin.

Trans means across. Femoral is relating to the femur. So I'd say transfemoral is across the thigh.
