There are alot of freebie booklets out there try BD @1-800-237-4554 or bd.com for a downloadable one. I just write it on a postie note and keep it on the back of my monitior. (remember to write the day and time along with your BS##.

Lifescan's OneTouch UltraSmart meter tracks 3000 readings as well as general health, meals and exercise. There is also a computer program that can download the meter readings for various reports - by week, two weeks, month, two months and 90 days. Plus there are different ways to report the findings, averages, etc. There are other companies that offer similar products; but, I would check out the UltraSmart meter first for convenience and its memory. Other meters hold a lot less - some only 300 previous readings and no computer downloading. Additionally, there are many websites that offer tracking tools for free or small fee (for programming) that tracks your progress and provides information or reports. A simple google search is all that's needed.

If you have a One Touch glucose meter they will give you one or two booklets a year to record blood sugars in for free. Go to their website and request one and they will mail it to you. It's about the size of a pocket calender. It's a handy thing to keep.

You can write it in a small note pad and make a column with date, time, blood sugar level and if you're taking insulin record the dosage you have to get.

If you are familiar with Microsoft Excel, that works pretty well for creating your own log. You can either just save it as a file or print it out.
