What a fun project!
Paramedics save lives, so how about Life Savers candies for a recurrent theme? The shape would make a great "circle of life" cake, and it suggests wedding rings. The peppermint flavor is white, and the fruit flavors come in plenty of colors to coordinate with.

How about getting some tongue depressors, kid's colorful band-aids, shot glasses, and any little candy that looks like pills. Fill the shot glasses with the "pills". With a colorful pen write across the tongue depressor "Niece and fiance - use their names getting married - AHHHHH" and tie it all together with gauze with band aid in center to make tie more colorful. Wrap the shot glass of candy pills with gauze and note that reads, "Have a pill and a shot on the bride to be"

Well first make sure if she would like it kept "bridal" or if she minds the theme of her job thrown in. You might use little Dr's bags full of mints (hearts) and a stethescope. A bit corny...but think on that. You'll come up with something great!
