By having an excellent standard in food, an excellent decor, excellent staff, superior wine list, excellent everything. Then the Micheline folk come to your restaurant "undercover" and rate you. (I'm sure you need to let them know that you are trying to achieve that standard- I'm not sure how that bit works). They do this every year and can give you stars or take them away.
There are also "red M's" which mean the decor may not be great or the location, but they think the produce, food, wine list is of a very high standard.

It has to do with the whole dining experience. The food's got to be perfectly cooked and fresh, perfect presentation. Also the menu needs to be innovative and creative. And the venue and surroundings need to be dapper, efficient staff, the works ....

They are no longer doing michelin stars - I think they are doing rosette's or something.

You wait for them to come to your place. Good luck I'm in the book and it was a surprise to me. Never turn down a table for one. that is my only advice

you have to cook very ordinary food so that it costs a fortune and looks like a hat!
