Good question. Simple answer. Herpes is painful. Syphilis is not painful at all. Herpes is super common, Syphilis is rare. Herpes has a bunch of tiny blisters at a time, syphilis is just one ulcer. You can do a simple blood test to check for syphilis. Herpes is diagnosed by looking at it and taking a sample right from the blister and sending it for a culture which is very accurate. I would say it is definitely possible, but unlikely if you have a competent practitioner. In males, herpetic lesions will usually be at the base of the penis and syphilis will be on the shaft or head. Also, in males and females, herpes lesions hurt and syphilis lesions do not.

Not by a competent physician...Visually they can tell but most often will get a culture to confirm their diagnosis.
