Type 1 or Juvenile onset is an autoimmune disease, possibly a small genetic component. Autoimmune means that the body's immune system destroys the insulin producing cells in the pancreas. Type 2 or adult onset has more of a genetic component than does type 1. The risk factors of type 2 used to be over age 45, obesity, and sedentary life style. Type 2 is now finding its way into younger populations because of childhood obesity, and a lack of exercise due to things like video games.

Genetics play an important role. However, in Diabetes Mellitus type 2, lifestyle is also an essential factor. The way and the kinds of food you eat and the frequency of your physical activity can tell a lot about your predisposition to having DM.

Depends on whether it's type 1 or 2. Type 1 is generally biological and can't be predicted or avoided, and usually happens in young age. Type 2 is usually found in older people with obesity, poor diet/exercise, etc. and can often be avoided. This is kind of a vague, general answer, but we only get 300 characters...

Some are genetically handicapped.
Some use too much insulin on a constant basis.
Some are just lucky like me.
