Elevated insulin levels are seen with:
1. Acromegaly

3. Drugs such as corticosteroids, levodopa, oral contraceptives
4. Fructose or galactose intolerance

6. Obesity

It means there is a lot of insulin in the body. The more important level would be C-peptide. Insulin begins as a precursor which gets cleaved. The result is insulin and C-peptide. C-peptide is measurable. If insulin is high and c-peptide is low, then it indicates an exogenous source for the insulin (ie, injected)
