It would be much better to find a contact person at Sears to whom you can address the letter - you can easily check their website for the contact or make a phone call if necessary to find a name. Otherwise, direct the letter to a specific department, such as "Dear Customer Service Representative" or "Attention: Regional Manager" etc.

Yes. Possibly address to "The Service Department" To the person in charge of (some other) Department. To whom it may concern is a lilttle vague.

Well if I am trying to be respectful I will use Sir or Madam. Like if I want them to do something for me. ;) Its a nice touch.
If I'm complaining about something then, To whom it may concern sounds more appropriate.

"Dear Sir or Madam" is kind of outdated.

Make it to the attention of whatever department you're writing to, and have the salutation be "to whom it may concern."
