Medicaid in most states will not cover anything that is not considered a medical necessity. Being that weight can be lost via other methods it is generally not considered a necessity.
I am not sure about the state you are in, but in alot of states you would go about applying by being referred to a Dr. that performs the surgery (and that takes Medicaid) by your family Dr. Then the surgeon will fill out all applicable paperwork and get a form back letting them know if it will be covered or not from Medicaid.
If in your state you don't need a referral just start calling around to different Drs that perform the surgery and finding out if any accept Medicaid. Usually the problem is finding a Dr. to take the insurance in the first place.

I found the book to be very informative and easy to read.
I've lost 17 pounds in 12 days, I'm just concerned that I'm losing too much weight, too quickly. I will admit I haven't followed the guide exactly. I'm not sticking perfectly to the listed foods and meal plans (but mostly) and doing very little exercise, but the weight keeps flying off.
It clearly works and if I'd followed it exactly I think it would scare the living daylights out of me because of the amount that I'd lose. So thanks again for the information. I've never purchased anything like this before because they are usually full of trash, but 3WD has been a pleasant surprise.
