Since she has everything, just give her some flowers, a birthday cake and and a dinner out if she can go out.

Depends where she is living. I know my great aunt is in a nursing home and has very little room to store anything. She appreciates chocolates, licorice and cream & jam sponge cakes. A food treat, something that the person loves is always nice. She also always appreciates soap & powder either "rose" or "lavender" since these are old fashioned fragrances that she grew up with, no point buying perfume, at their age, they wouldn't think to wear it, but soap & powder are useful. They just use the cheap stuff at the nursing home so she loves these treats as well since they make her feel special. It might seem a bit boring to you, but people of this age lived through the depression and they have never been in the habit of buying things like this for themselves so it will definately be appreciated.
We also have another old friend who's 92 & still lives alone in her own unit. (she's so fit & healthy for her age, she looks 70!) She loves her garden & appreciates pot plants, just something small that fits into her courtyard. She loves flowering things & says it reminds her of the person that gave it to her. She's got about 100 of them but she says it keeps her busy.
No point buying clothes or handbags, people of this age aren't really interested in their appearance anymore, and the chocolates, soap, etc. are usually much more appreciated. I bought my great aunt a new handbag once because her old one was about 20 years old and her reply was, "What do I need that for? I've already got a perfectly good handbag."

Maybe some perfume or a nice piece of jewelry. There are websites that have gift ideas for all ages. The names escape me now but you could google 'gift for 93 yr old female'

Just recall your common happy moments of the past, the priceless treasure that has cemented your relationship with her. That is a gift of God which you can present her and she will appreciate with tears of bliss.

Maybe you could compile a photo album of pictures of you and her. I also like the suggestion of having her portrait done.
