Am I right in thinking that it would be when the visibility is bad? And when would it be denied? I bet the drivers are kept busy in places where fog is common...

Also when an aircraft has been diverted to an airport that the crew are unfamiliar with and have no airfield map.

It's probably more commonly requested if the Pilot is unfamiliar with the airport and doesn't know where his terminal is. If the fog is bad enough that the pilots taxiing can't see the runway and taxiway lights the airport is probably locked down anyway.

I might add that if you fly into most Russian airports you will be required to follow a "follow-me", both taxiing in and for that matter taxiing out as well, no matter what the weather. And you'll get an earful if you blind the driver with your landing/taxi lights. Just as well, really, considering the habit of parking you wherever convenient instead of according to a prepared plan.

Yes you are right in bad/foggy weather. They also follow when they are not familiar with the airport or going somewhere that has not been updated on their map. They also follow when there radio is not working.

well when visability is low they may ask or if a pilot is new to the airport they might request or lots of snow, but its rare these days with the new technoloy in lighting and plates

What? Are you talking about an escort aircraft?

If so, it's normally when the pilot suspects the plane may be about to fall under the control of someone else. OR when the plane loses radio contact.

If you mean on the runway / taxiway then yes, sometimes when the visibility is bad they will have follow vehicles although it can slow things down a lot.

I've flown through SFO 6 times this year (4 times in heavy fog) and not once have we had a follow vehicle.

yep if the fog is bad like places in SFO(where its foggy often) or whatever them may have a follow me car or even have a tug if the weather conditions/visibility is bad. it would probably be denied if they weren't needed (wasn't any fog).

When i said SFO i didn't mean they always had one. smaller airports may have these follow me vechile especially if there isn't runway lights/taxi lights or it's an uncontrolled airport.

Yes, that is correct. Also in places where there is a lot of congestion and the ramp is not marked clearly (Denver uses FollowMe trucks on their de-ice pads to make sure every plane is exactly where they want it)
