This can only be answered by a vet. There could be many things wrong, or it can just be a birth defect. Taker it to the vet for an evaluation, and all your questions will be answered.

If there is no drainage which would indicate an infection, the other possible cause may be head trauma, cataracts from old age, or a hereditary condition "cokes disease".

Could be bad breeding, could have been an accident in the whelping box.. Could be all sorts of different things. Take him to the vet and find out what's going on.

There can be underlying problems causing this so please have it checked as soon as possible
