I received a print-out page of a traffic ticket in NY on Route 684 for driving over a divider without any signage. It is not off an original copy and all of my information was typewritten. Furthermore, the ticket was NOT SIGNED, does not contain any court address, and does not include a phone number or source of contact. Essentially, I have to perform due dilligence in handling the matter. Is this ticket legitimate or did the officer just give me some kind of warning with a print-out copy of a ticket?

No - this is a ticket.
If there is a phone number on it, contact the court for additional information.
If not - appear in court on the date specified. You may ask the Officer in court about the nature that the ticket was presented to you, and any other irregularities you feel should be presented to the judge.
However the judge will have one question for you. Did you drive over the divider?

Talk to a judge on the date on the ticket to fight it or pay it. If you don't respond and it is a real ticket they will issue a warrant for failure to appear.

That sounds unusual. However, many departments in my area have gone to a computer printed citation. It could be an instruction page is missing.

The only one that can answer that question is the clerk of courts in the jurisdiction you were 'ticketed' in. Call them, or better yet, bring it down to them.

You don't want to end up with a suspended license, or a warrant, for someone else's mistake.

Until a court tells you it is not a ticket you should assume it is.
