You already know what to do - you're aware what are not healthy foods, and that you've been eating them. So you probably know what are healthy foods, too. What you need is the motivation to eat mainly healthy foods, and keep away from junk.
Different methods work for different people. If you like a structured plan, you could make up a meal sheet for each day, and stick to it. Or you could simply avoid all high-fat and high-calorie foods other than one day a week when you treat yourself to chocolate (or whatever you crave).
Do keep away from fast-food places, those are the worst - even salads at those places are generally covered in high-fat dressings. Make sure you eat regularly, and get plenty of fruit and veg, and drink lots of water. Avoid artificial sweeteners and anything with hydrogenated fat, and go easy on anything with sugar or other fats. Eat wholegrain products wherever possible, as they fill you up more and provide more nutrition.
You're very sensible to take control of your eating at 15 - if you can develop healthy eating habits now, it will stand you in good stead for the rest of your life.
Remember to take some exercise too - it doesn't have to be strenuous; even twenty minutes of brisk walking four or five times a week can help.
