I have a Ford Windstar minivan with two captain chairs and a 3rd row bench. I have 2 toddler carseats installed in each captain chairs. My 11 yr old sits on the 3rd row bench.

I have a minivan with built in child seats.. they have sat in the middle row.
I think that is the safest...

What I have done is have them in the middle captains chairs normally. When we have other kids even teens I make them go to the very back and leave my kids as is for the ease of doing up car seats etc. When we have adults, my in laws (ones with knee or back problems like you stated) then I just move my kids to the very back, its a pain yes but is the respectful thing to do and give them the middle seats. Or we let them drive and we take the back seats. Actually I do this with any adult not just the ones with physical troubles.
I would just take out the small middle seat in the 2nd row it makes easier access for anyone trying to get to the very back. Both my husband and myself have gotten back there fairly easy but we do not have the small seat in between the captains chairs, I would definitely remove that regardless of who was trying to get to the very back.

My friend has triplets and needed to place 3 car seats in her minivan. She removed one of the captain seats from the middle row. She put 1 car seat in the other captain seat in the middle row and put 2 car seats in the rear row. With the middle row captain seat missing, she had very easy access to the back row and all 3 kids fit perfectly in her minivan.

I have an Odyssey
I have my 2 kids' car seats in the second row.
When I have my elderly relatives visit, I put one in the front set, and I assist one to the back seat.
Not perfect, but since I am travelling with the kids daily, it makes most sense to make it most convenient for everyday travel.

I have a 00 oldsmobile shilouette, and i have the two captians chairs in the middle. Thats where I put the carseat, and when you do have the other adults in the van, put the other carseat in the back.

I drive a '96 Voyager. It has standard seats(only 2) in the middle, but I woudl put the children in teh same place with your captains chairs. I have my toddler closest to teh door and my infant behind the driver. I had them the other way aroudn at first but found it VERY difficult to access my toddler in a hurry when needed. It is MUCh easier to get around the toddler seat to the infant seat than the other way around.

I would go with the second row since you will mainly be transporting your children. You could always move one of the car seats and have the other adult sit in the front when they are with you.

I would place the rear facing chair on the small seat and the toddler chair on the 3rd row close to the door. You have quick access to both children. This also allows room for your other passengers. One in the passenger seat up front. The other in one of the captain chairs.

In the middle captain seats. Its usually those that are equipped with the latch system because its that area of the van that is the strongest and safest.
Beyond that the van was designed for easy use for mothers with car seats by putting those seats at the openings for the doors, so you're not climbing in and out all the time.
