It is metaphoric. Jesus is saying that if things in your life lead to you sinning, you should remove them. For example, you might have a habit of going out getting drunk every night. So cut off that practice. Remove those habits that lead you to do that.

In Jesus mystic schooling teachings it was taught The Eye is the leader of a group. A Right eye would signify a Righteous Leader of a Group. And if this leader is evil, being full of hypocrosy, then that group leader needs to be replaced.

Jesus wants us in heaven with Him. We should do what we have to keep His commandments even if it means something extreme. Jesus will give us back the hand or eye. Nothing is more important than obedience to Jesus.
