In my dream I am in a big building with lots of people I don't know. All of a sudden explosion and fire break out, and my boyfriend is killed.

In my dream I don't get upset, and then an old (male) friend starts following me around, and trying to take the place of my boyfriend.

He then walks across a road near a place familiar to me, and intentionally slows down so a car will hit him. The car doesn't stop moving for about 300 metres down the road.

I then call an ambulance but I can't hear the woman on the other end, and i am too upset to speak very well.
An ambulance arrive, and i rush to see my friend, who has now turned into my boyfriend. He gets up and says he is fine.

When I woke up I felt terribly upset. I felt really appreciative of my boyfriend and just how much I love him.

Could this dream be about realising how important he is to me? Or something else?

Your dream indicates a fear of losing your boyfriend. Maybe your boyfriend and you are exposed to pressure or trouble from the outside world, that your boyfriend might not be able to handle as well as you, and you may lose him in some way. You may not realyze enough what he means to you, so you are not upset at the time he dies in the dream. Or maybe on some level you realize that your in a dream so you don't get affected as in real life. You replace him in the dream, but this does not work, the replacement gets in trouble to. But when the replacement is changed to your boyfriend, all is well again. Your boyfriend does seem to be more important to you than you may realyze, and not easily replaced. You may need to put more effort into making sure the relationship stays strong. Also, it appears you may need to protect him a little from getting into situation that could be more than he can handle. Meditation/prayer can help to keep both you and him safe. Exercise and a healthy diet can help one handle more, and also have more energy for the benefit of the relationship.

Death is rarely a bad omen. Dreaming of being dead means the end of your worries. and the start of a new era in your life. Speak to someone who is dead and you will soon get some good news. Someone else's death can mean a birth or a small warning for you to slow down as you are taking on too much. The death of an absent friend predicts a wedding. Be aware of bereavement, and news of a birth, engagemnet or wedding is on the way. Happiness is in the cards if you see a dead person a float.
