quarterly tax form for independent contractors?

I started working part time as an independent contractor in September. Can someone tell me what quarterly tax form (federal) do I need to fill out. I want to pay my quarterly taxes.

For federal, 1040ES. For state, depends on the state - check the department of revenue section of your state website.
