When does forgiveness become permission to re-offend? When does demanding accountability for someone's actions become unforgiveness?

forgiveness, in the sense of not punishing for an offense, is a different matter from restoration of trust

If I steal money from you and then being sorry I ask for forgiveness and say I'm sorry you would be right to forgive me. If I just keep stealing from you and merely say I'm sorry without stopping the stealing you would know I'm a liar and my asking for forgiveness is just a sham.

You don't. Harboured unforgiveness can't be forgiven unless you get rid of it by confessing unwillingness to forgive unconditionally. Lord's prayer 'forgive us our sins as we forgive those who have sinned against us'.

imho... forgiveness requires accountability.

If someone is truly sorry for something they did, and sincerely promises to improve, forgiveness comes easily and naturally.

If someone does something wrong, and does not feel remorse for it, forgiveness generally doesn't happen and mistrust forms.
